Spiritual Care Department

 Mission

In keeping with the Mission Statement of the Hospital,
the
Spiritual Care Team
commits itself to
holistic spiritual ministry.
We will strive to provide
spiritual and emotional support
to each person entrusted to our care,
while respecting
the diversity of the individual’s faith connection
for the enhancement of their
quality of life.  
 

Patient Care

The Spiritual Care Team participates in:                                 

         One-on-one individual ministry

         Ongoing support for patients, families and friends

         Ongoing support for staff

         Coordinating local clergy to lead worship

         Church services for patients, families and friends

         Ongoing spiritual assessment.

         Contacting patients’ own clergy

         St. Lawrence Parish Ecumenical Communion Services

         Gospel Reflections

         Seasonal Prayers

         Roman Catholic Mass

         Praying the Rosary

         Individual Ministry

 Monthly Church Services – Charles Street Site

         Ecumenical Service

         Praying the Rosary

 Spiritual Care Team

This team consists of:

         Spiritual Care Coordinator  

         Spiritual Care Trained Volunteers and Chaplains

                        Providing one-on-one pastoral ministry to those in need.

Assisting with portering of residents and patients to programs and services.

         Staff Chaplain

                        Providing pastoral visiting services on both sites.

         Community Clergy

                        Visiting their own parishioners and leading ecumenical services.

 Individual Support

         Trained team volunteers visit residents and patients. Individual visits are scheduled to meet the needs of the person being visited.

 End of Life

The Spiritual Care team:

         Offers support, comfort, and prayer ministry, prior to, at the time of, and following the death.

         Journeys with anyone who may be involved with the dying patient.

         Contacts and is in liaison with the patients own Clergy, when applicable.

 Bereavement Follow-Up

         Follows up with the bereaved family by attending wakes or funerals, whenever possible or by sending on-line condolences.

         Sends a letter of concern, 6 months after the death then an invitation to the Palliative Care “Annual Celebration of Life” service, to all the Complex Care residents’ bereaved families.

         Offers “Time to Exhale” Sessions for staff dealing with the stressors of multiple losses.
